The Collaborative for Children and Families (CCF), is honored to be given the opportunity to be partnered with the CANS-NY Technical Assistance Institute. Supported by the New York State Department of Health (DOH) and Chapin Hall at the University of Chicago, the Institute provides training, coaching, and technical assistance in effective use of the CANS-NY as a collaborative assessment and treatment planning / monitoring tool for Health Home Serving Children program in NYS. In addition, The Institute uses data analytics to guide and support the use of the CANS-NY to transform the lives of children and families through Health Home care management.
CCF has hired Ms. Cynthia Schelmety as our lead training coordinator and lead coach for the Downstate NY region. We cover all five borough of NYC (Manhattan, Queens, Brooklyn, Bronx and Staten Island) and Long Island (Nassau and Suffolk). As a lead regional coach Ms. Schelmety provides training, technical assistance, and support in integrating the CANS-NY into care management and supervisor processes.
