As part of our Community-Based Services Division, the Collaborative for Children and Families (CCF) is proud to serve as a Social Care Network (SCN), helping Medicaid-enrolled individuals and families access critical social supports.
If you or a family member are enrolled in Medicaid and need help with food, housing, transportation, or other essential services, you may be eligible for assistance through our SCN.
Our trained Health-Related Social Needs (HRSN) Screeners and Navigators work directly with individuals and families to identify specific needs and connect them to community-based services tailored to their health and circumstances.

Children and Family Treatment and Support Services (CFTSS) are Medicaid services designed to provide mental health services to youth with social, emotional, or behavioral health challenges and/or substance use issues. This strength-based program serves youth between the ages of 0-21 years with NYS Medicaid. CFTSS gives youth and their families the power to improve their health, well-being, and quality of life. These services strengthen families and help them make informed decisions about their care. This program is certified through the New York State Office of Mental Health. Enrollment in the program is voluntary and paid for by Medicaid. Providers meet with children, youth, teens, and their family members in their home and other locations of their choosing.

Eligibility Criteria
Children, youth, and teens ages birth to 21 who are enrolled in Medicaid and who need help with social, emotional, or behavioral health challenges, or need help with substance use issues.
Access to Services
To receive CFTSS, a youth will need a referral and a recommendation.
Referral:
Anyone can make a referral to a CFTSS provider for a child/youth they believe needs services(s)! A youth who thinks they need CFTSS can even make their own referral to a CFTSS provider.

What can I expect from the CFTSS workers?
CCF will work with each child/youth in a way that works best for them and their families! At CCF we put the child/youth and families’ choices first. These services are delivered at the home or in the community where the family is most comfortable. The goal of these services is to help find and treat children/youths health problems early so that they can have the best health and development possible for youth who need help with social, emotional, or behavioral health challenges, or need help with substance use issues. Each child/youth is supported throughout services in order to strengthen families and help them make informed decision about their care.
CCF’s services are distinct, individualized, family-driven, and are designed to empower child/youth and families to enhance their quality of life. CFTSS providers offer a comprehensive range of support options designed around the specific needs of the children/youth.
